To return an item by mail, please follow these steps:

1. Please fill out this form to obtain a Return Merchandise Authorization Number (RMA number).
2. Box up the item(s) you’re returning including all tags and original packaging that came with the product.
3. Write the RMA number you will receive via email on the box.
4. Please include a copy of your receipt, or invoice, if you no longer have one of these, a letter with the following information:
   • Name
   • Address
   • Daytime and Evening Phone numbers
   • E-mail address
   • When, where and by whom the purchase was made
   • Reason for the return
   • Purchase price of the item(s)
5. Please specify whether you would like an exchange, or a refund.
6. For your protection Insure the package and obtain a tracking number from your shipping carrier.
